"Repair using FUSE - 2003 to now Pics- what will you suggest?"


          

Following are some pictures of a patient with an interesting pattern of hair loss and his journey to the present day.

The patient is a Sikh gentleman and used to wear turban for many years.

As a result he developed traction alopecia- loss of hair resulting from constant pull accompanied with pressure in form of turban.

He initially went to some other doctor, where he was promptly administered rows of minigrafts and plugs in the front hairline and the temple areas.

Dismayed with the results, he convinced the original ht doctor to excise the plugs in the hairline. The pluge were excised in form of a strip which was sutured closed.

The temple area minigrafts, however, remained.

That was the time the patient initially approached us- in 2003.

There are many approaches to treating the minigrafts in the temporal areas.
We followed one path. But I am sure alternatives exist.
I would request you be in the shoes of the patient, (or of future ht doctors) and think and suggest what possible treatment options could have been offered.

Let me briefly outline the problems in this case.

The temple areas right down till the back of the ear had lost hair and were sparsely transplanted using minigrafts(5-8 hair) and plugs(10 hair/graft).

The plugs and the minis were "standing out". That is they were wrongly angled and pointing outwards rather than acutely downwards.

There, of course, is the stitch scar in the hairline area.

And there are some hair in the hairline area that give an unnatural appearance and the patient has to shave them to make them less obvious.

this s the first time i heard plugs repair done by doing a strip excision in the hairline.

Is this particular patient still wearing a turban? If not, how long has he been without? Traction alopecia, may or may not be a permanent fixture, and could have the possibility for some regrowth, dependant on current tension factors. If this is not applicable in this case, the patient should proceed with plug removal where absolutely necessary, in which camouflauge is near impossible. It appears that the temporal/sphenoid, and possibly anterior parietal areas require absolute elimination, while the frontal hairline may be FUSE'd and grafted throughout the linear scar tissue in an attempt to conceal. The minigrafts on the ridge of anterior lobe could remain if the fresh recipients are beyond the horizontal scar.

*ALL ten haired grafts should be removed, as they are extremely difficult to mask and visually excessive to the eye....

It is unfortunate that this patient was afflicted with Traction Alopecia, as he does not seem to suffer from MPB symptoms whatsoever...Keep those caps loose guys!

What did you do for the strip scar in hairline and the unwanted hairline hair?
I would advise that the unwanted hair be fue'd out and planted into the strip scar in hairline.
Providing fueing the hair from hairline does not give any scarring or white dots.

The before picture is kind of fuzzy, but good enough to demonstrate the problem in the temple area. Nice work Dr. A! This pic and its "after" really highlight your skill. Here you've achieved great growth and good coverage in a scarred area with 200+ grafts and some BH to boot. I feel encouraged. Why has this particular result received such small kudos so far? Thanks.
