"Hair restoration for a Norwood 6 (photodiary of surgery)"

Allow me to share an ongoing surgery details with you.
The patient is a NW 6 Caucasian.
The approach is to use scalp donor hair and an assortment of various body donor hair to accomplish this stage of his hair restoration procedure.

FUSE procedures with inclusion of body hair grafts has allowed fuller hair restoration procedures possible.

This will be an interesting case. His scalp donor (sides) is thin. One thing he has going for him is scalp hair contrast. It looks like his chest hair is darker than scalp?? Is that true?

Dear Checkingin,
Yes, the scalp hair is light in color and that will help by lowering the scalp skin and hair color contrast.
On the other hand, a lot of the chest hair is darker, so it will provide better coverage.
Lastly, the facial hair is mostly white but of a thicker calibre.

We have decided not to use the thigh and leg hair in this patient as it is thin as well as light colored.

The graft numbers has also been kept open ended for the present.
Though there is an anticipated number, however, the focus will be to aim for better quality hair rather than numbers per se.

Dear forum readers,
The first day of the surgery commenced with transplanting a bridge of hair joining the two sides on top.

Hair grafts were extracted from various donor areas and were inserted back, with little time gap/ almost simultaneously, into the pre prepared recipient sites.

The grafts from different areas were transplanted in a predetermined mix.

Mixing the donor hair from different areas helps avoid any type of hair from looking too conspicuous.



Total number of grafts - 1865.

These include 575 scalp, 431 abdomen, 423 chest, 315 beard and 121 armpit grafts.


The surgery commenced at 9:15 am (start of anesthetic application) and finished at 6pm (wrapping the recipient area in bandage).

Starting at 9:30 am the procedure finished at 6:30 pm.

A total of 1537 grafts were transplanted.
1035 scalp,
320 abdomen,
143 beard and
19 back hair.

The hair were transplanted in the frontal area (not the final hairline zone).

It has been my endeavour to present pictures that "leave nothing to imagination".
Pictures that gives the propective patients a true idea of the surgery and its following days.

However, I found some interesting features while taking the following pictures of the scalp donor area.

Please view them and let me know which picture you think conveys the most real feel.

The first one is without flash, with what I consider inadequate lighting AND with the neck flexed backwards at a normal angle.

The second one is with flash with the neck in the same normal flexed back angle.
The third is with proper illumination (in my opinion - adequate light but no flash) AND with the neck flexed FORWARD.

All these pictures are immediately at the end of the day's procedure.

To me the middle shots, flash with neutral neck position, show the best detail by far. The first, which you said was no flash (?) was the least helpful by far.

Hairline placement is a choice that the doctor should make keeping a patient's reasonable wishes in mind.

The present patient, whom, for convinience sake, we shall refer to by the name Yorker, has 2 options.
A very conservative and high hairline v/s a moderately ambitious one.

As the pictures show, Yorker has a high and wide forehead.

The two lines marked give a rough indication of the 2 hairlines.

The upper ones leaves the temple angles open but frees more hair grafts that can be used in the crown.

The lower one, with the temple angles closed, means that the density in the crown and whorl will be less.

Here are some pictures.












I hope introspection will help future patient decide on the type of hairline they require for themselves.

This case is one I will love to follow up. Its amazing. This gent is more bald than anyone I have seen. Seems like a norwood 1000 to me.

Still, in his shoes, I will choose the lower hairline. Looks way more pleasant and its not really too low. As long as there are some hair smattering on the crown, it should work.

Day 3 included a total of 2039 grafts.
The patient, Yorker, preferred the lower hairline.

The grafts included 1123 scalp, 562 chest, 202 abdomen and 152 beard grafts.
The front 1 cm of the hairline had no facial hair grafts.

This is just what I have been saying. You see all the doctors saying 3000 or 4000 for a really bald guy.
Now 6000 is finished in just part of the front.

Dear forum readers,
Hair were extracted from the scalp, including the nape area, the sideburns, chest and abdomen.

Day 4 -
Total 1357 grafts (509 scalp, 429 chest and 419 from the facial areas).

Day 5 -
Total 1456 grafts (592 beard, 426 chest, 266 scalp and 172 grafts from the abdomen).

With these grafts, the hairline/frontal areas were completed and work performed over the top going to the crown.

As we near the completion of harvesting most of the robust body donor hair from the chest and abdomen in this patient, the option in front is to go for the less robust hair or to reharvest the previous body donor areas.

Hair are constantly entering the growth phase. Thus, the body donor areas, from where the growing phase donor hair were extracted 4-5 days earlier now have some more extractable and robust donor hair.

We feel it is better to use this option that to go after the less robust hair for filling the top and the crown.


Apart from these donor hair, we will also be using the remaining nape/neck hair and the beard hair.

It was, thus, that 1024 grafts (402 beard, 374 chest and 221 scalp grafts), were extracted on Day 6. These were used to recreate the swirl and fill in the crown areas.

At the closing day of the procedure, atotal of 757 grafts were transplanted (457 beard donor hair from lower neck and 300 abdomen hair grafts). The abdomen hair werereharvested from the previously harvested donor to aim for the more robust hair.

Total number of grafts for this procedure - 10,035.

Of these, 3729 were scalp hair grafts
2480 were facial/beard hair grafts and the remainder from chest, abdomen and armpit.

That is roughly 25% from the beard and 37% from the scalp.
